Student Housing Questionnaire (SHQ)
ATTENTION parents, legal guardians, caregivers, and unaccompanied youth: The purpose of this questionnaire is to help identify school-aged children and youth who are experiencing housing instability as defined by Subtitle VII-B of the McKinney-Vento Homeless Assistance Act (42 U.S.C. 11431 et seq.).
INSTRUCTIONS: All families must complete this form. If you own or lease your own home, you only need to confirm that below. Families experiencing housing instability may qualify for McKinney-Vento services and resources through the HEART program.
